You will need to submit a 2014-15 application.  It is necessary for your student to continue to receive Free/Reduced Breakfast and Lunch in the 2014-15 school year.

2014-15 FREE/REDUCED LUNCH APPLICATION

 

 

If you receive a Direct Certification from Social Services, you do not need an application, we do need a copy of the Direct Cert. or your food stamp number.  

Applications are to be returned to Hilton Food Service, 400 East Avenue, Hilton, NY  14468.  We cannot accept a faxed copy we must have your original signature.

If you do not qualify in the beginning of the school year, and your need changes, you can apply throughout the school year.

1)  Q Do I need to fill out an application for each child?
A
No. Complete one application per family to apply for free or reduced price meals.
2)  Q Who is eligible to receive free meals?
A
Children in households getting Food Stamps or TANF and most foster children can get free meals regardless of your income. Also, your children can get free price meals if your household income is within the free/reduced limits on the Federal Income Guidelines.
3)  Q Who is eligible to receive reduced priced meals?
A
Your children can get low cost meals if your household income is within the reduced price limits on the Federal Income Chart.
4)  Q I was approved for free/reduced meals in May is the application good until next May?
A
No. The application is only good for the school year. A new application is needed for each school year.
5)  Q I receive WIC, will my child(ren) get free meals?
A
Children in households participating in WIC may be eligible for free or reduced price meals, but an application must be completed.
6)  Q Will the information I give be checked?
A
The school may ask you at any time during the school year to verify your eligibility. You will be notified, in writing, if you have been selected for verification. School officials may ask you to send papers showing that your child should receive free or reduced price meals at the time you applied.
7)  Q If I do not qualify now, may I apply later?
A
Yes. You may apply at any time during the school year if your household size goes up, income goes down, or if you start receiving Food Stamps, TANF or other benefits. If you lose your job, your children may be able to get free or reduced price meals.
8)  Q What if I disagree with the school’s decision about my application?
A

9)  Q May I apply if someone in my household is not a U.S. citizen?
A
Yes. You or your child(ren) do not have to be a U.S. citizen to qualify for free or reduced price meals.
10)  Q Who should I include as members of my household?
A
You must include all people living in your household, related or not (such as grandparents, other relatives, or friends). You must include yourself and all children who live with you.
11)  Q What is considered household income?
A
Eligibility is based on total gross household income. All income of household is considered when determining eligibility, including child support, alimony, social security benefits, workers compensation and net rental income. Any child eligible for free or reduced price lunch is also eligible for free/reduced breakfast. Reduced price meals are $0.25.
12)  Q Can I fax a copy of the free/reduced application?
A
No. We must have an original signature, copies are unacceptable.
13)  Q If I am denied, may I reapply if my situation changes?
A
Yes. However you must submit supporting documents to be reconsidered.
14)  Q What if my child is approved for free lunches, however she does not want a full meal, may she receive milk only for free?
A
No. In order to be eligible for free and reduced meals, all meal components must be taken, otherwise the meal is considered alacarte and will be charge accordingly.
